Please write the name as on the tax certificate. For Turkey enter a 10-digit VKN; Look up can fill name, tax office and address. If this company VKN is already on file, that is fine — each contact opens an account with their own email. Add trade registry no. and founding date if you have them.

Please write full names for GM and purchasing. Separate partners with commas. Account manager is your Leras contact if you know them; otherwise leave blank.

Activity e.g. innerwear manufacturer, wholesale apparel. Payment e.g. Peşin (cash).

Please write the official company address. Phone and email are how accounting will contact you; the account is created with this email.

Please write the bank name, branch and account number (IBAN is fine). Use Add another bank for extra accounts.

Please do not write your own company name. List three suppliers you buy fabric from. Each row needs the supplier’s name, a reference person there, and their phone. In the note, briefly describe how you work together.
